Before there were electric drills, or drills that were powered by water pressure or air, they used a treadle drill. Like a treadle sewing machine, the doctor would have to work a pedal to make make the drill spin. And it was slow, distracting, and not really all that constant.
It's why people fear the dentist, well one of the reasons anyway. Luckily this child is getting a tooth cleaning, but that brush looks rather large.
